In accordance with NYS law, the District will provide employees with up to four (4) hours of paid leave time each calendar year for the the purpose of obtaining breast and/or prostate cancer screening. IF YOU INTEND TO OBTAIN THIS CANCER SCREENING DURING YOUR NORMAL WORK HOURS, YOU MUST OBTAIN THE REQUEST FORM FROM THE MAIN OFFICE AT LEAST ONE (1) WEEK PRIOR TO YOUR DOCTOR'S APPOINTMENT. To obtain your $100.00 Medical Reimbursement you will need a Discretionary Benefits Fund Reimbursement Form. The forms are available on the District website, look under the Staff heading, then Forms & Documents, and finally Payroll.
